The first call to a care provider is a free, no-obligation conversation. You do not need a diagnosis letter, an assessment report, or any idea of what the support should look like. Nothing is agreed and nothing is signed. We talk through the person you are calling about, what is happening now, any clinical needs, and how the funding might work. Then, only if it feels right, we take a next step at your pace.

For most families, that first call is the hardest part of arranging support. It is harder than the paperwork or the funding. By the time you pick up the phone you have usually been holding a lot together for a long time, and explaining it all to a stranger can feel overwhelming. So here is what actually happens, with no jargon and no pressure.

You don't need anything prepared

Plenty of people open the first conversation with "I don't really know what I'm asking for." That is fine. Working it out together is what the call is for.

It is free, there is no obligation, and nothing is agreed or signed.

What we'll talk about

We start with the person rather than the needs. Who are they? What does a good day look like? What matters to them, from routines to interests to the people around them? Personalised support starts with the person, so that is where we start too.

Then we look at what is happening now: who is providing support today, what is working, and what has become unsustainable. If a family member has been carrying most of it, say so. There are options many families don't know exist.

We ask about health and clinical needs plainly. If there are clinical tasks involved, such as medication, seizures, feeding or breathing support, we will ask about them directly. Our service is nurse-led, so complexity doesn't frighten us. It is what we are built for.

We also help you make sense of funding. There are three routes that might apply: NHS Continuing Healthcare, Personal Health Budgets, and Direct Payments. We will explain which could fit and what the next step looks like for each. If you are already mid-assessment with a health board or local authority, we can work alongside that. What happens after the call

If it feels right to go further, the next steps happen at your pace. That means a fuller assessment with one of our nurses, a conversation about who you would want in your support team (with The HCD Model, you choose), and a plan covering training, funding and start dates. At every stage you can pause, ask questions, or decide it is not for you.

If we are not the right fit for your situation, we will say so and try to point you somewhere useful. That honesty costs us nothing and can save you weeks.

Three questions worth asking any provider

Whether you talk to us or anyone else, three questions will tell you most of what you need to know.

Who will actually turn up? Named people, chosen how, with what consistency?

Who is clinically accountable? Which qualified person assesses, trains and reviews the team, and how often?

What happens when things change? Needs evolve, so ask how the package evolves with them.

A provider comfortable with those questions is worth talking to. One who dodges them is telling you something.
